The form was acting goofy for me. I typed in my city which is 2 words and I kept getting an error saying "enter letters only". I had to take out the space between the 2 words. Then, I selected "other" as my provider and it wouldn't take it. I had to select one of the other ones. Oh well, they finally took it.
