HOW TO ENTER: To enter, during the Promotion Period, visit www.doctoroz.com where you will find out more about the promotion and where you will be redirected to www.lifelinescreening.com/know-your-5 and follow the directions to complete and submit the online entry form or call 1-888-576-LIFE and speak to a Life Line Screening representative. Only the first 5000 eligible entrants (as determined by Life Line Screening) to enter beginning at the start of the Promotion Period are eligible to receive a Health Screening. Your odds of receiving a Health Screening depend on when you submit complete entry information, including your full name, complete address and daytime phone number whether online or to a Life Line Screening phone representative. The database clock of www.lifelinescreening.com/know-your-5 shall be the official timekeeper of the Promotion. Two entrants per household, e-mail address, and/or phone number.


3. HEALTH SCREENINGS: Life Line Screening will determine, in its sole discretion, who were the first 5000 eligible entrants to enter the Promotion beginning at the start of the Promotion Period, and Life Line Screening will notify those potential recipients by phone or email. Those first 5000 eligible entrants, provided that they comply with Life Line Screening’s requirements, will receive the opportunity for a Health Screening. The Health Screening will include screening the following for eligible recipients: cholesterol panel, glucose test, blood pressure, weight and waist circumference. Each Health Screening has an approximate retail value of Eighty Dollars ($80). No substitution, partial acceptance, transfer, or cash redemptions in lieu of Health Screenings are permitted, except in the sole discretion of Life Line Screening, which reserves the right to substitute a different product or service of approximately equal or comparable value in the event that the offered Health Screening, or any portion thereof, becomes unavailable or impractical for any reason whatsoever. Other than the Health Screening itself, each recipient is responsible for all costs associated with obtaining the Health Screening (including but not limited to transportation and parking) and all costs associated with or resulting from any conditions that may be identified by the Health Screening. The Health Screening must be scheduled by December 31, 2010 and completed by January 31, 2011 or else it shall be forfeited in its entirety. Each recipient of a Health Screening is solely responsible for the reporting and payment of any applicable federal, state, and local taxes or fees resulting from or related to acceptance or use of the Health Screening, or any portion thereof. In the event that fewer than 5000 eligible entrants enter during the Promotion Period, fewer than 5000 Health Screenings will be provided.
