just tried it again and it worked fine - click on the link above, then skip the intro and then there is a little box that appears in the middle of the screen that says something about taking a survey click there and answer the 2 questions. then you get a form - i answered no to both and got the form for the crest with scope.

Not working for me either. I took the quiz, it comes up and says which sample I'll be getting but when I continued, I got this message.
